Welcome to the ACT Now Toolkit user guide.

This guide will explain how you can use this online toolkit, in collaboration with a survivor of stroke, to develop programs and habits that improve their long-term physical activity.

The flowchart

The 'Using the Toolkit' flowchart outlines the path you will follow through the online toolkit. Your answers to questions as you progress through the toolkit determine this path.

 

You can view the toolkit process by clicking on the flowchart image adjacent, or the 'Flowchart' button below, wherever it appears throughout the toolkit.

The flowchart can also be downloaded as a PDF via the 'Flowchart' download button on the 'Toolkit' page.

Registration

To access the online toolkit, you first need to register

 

You must provide responses for the 'Position,' 'Intended Use,' 'State/Territory,' 'Postcode' and 'Email' fields to successfully register and proceed. Additionally, sharing your name and organisation's name would be appreciated to gather feedback about the toolkit.

The Toolkit

As you navigate through the toolkit, you'll respond to questions, using buttons like those below, about the survivor of stroke. Your responses will determine your pathway through the toolkit. 

To aid discussions and planning with the survivor of stroke, you'll also download worksheets as you progress through the Toolkit, using a download button like the example below.

    Review and update Action Plans and create coping plans if needed.

    Identify necessary changes and support for survivors of stroke facing complex challenges.

Learn more about the project, how the toolkit was made, and the research helping health professionals promote physical activity for survivors of stroke in the 'About' section of the website.

This section can help you get a better understanding of why staying active after a stroke is important, as well as explain to the survivor of stroke why and how you're encouraging them to be active.

 

You will also discover the COM-B model in the 'About' section, which is the basis for many of the resources that come after.

Find helpful links, stroke information, self-management strategies, and exercise resources to improve survivors of stroke’s well-being in the 'Resources' section of the website. The references cited throughout the website are also listed on the 'Resources' page.
